Clinical Study on Toxemia of Pregnancy

Abstract
|
|
|
In spite of the widespread advances in the treatment of toxemia in recent years, it remains the major cause of maternal and perinatal morbidity and mortality. But good prenatal supervision, with the early detection of signs and symptoms of on coming preeclampia, and appropriate treatment will arrest many cases and sufficiently ameliorate others that the outcome for baby and mother is usually satisfactory.
The data tabulated are based on 318 cases with the diagnosis of toxemia among 2,210 cases of total deliveries between Apr. 1, 1968 and Mar. 31. 1972 at the Seoul Red Cross Hospital.
The results are follows;
1. The incidence of toxemia is 14.4%, preeclampsia; 11. 1%, mild preeclampsia;.3.3%, severe preeclampsia; 7.3%, eclampsia; 3.3%, antepartum eclampsia; 1.8%, intrapartum eclampsia; 0. 9%, postpartum eclampsia; 0. 5%.
2. The incidence of preeclampsia in toxemia is 77.0%, mild preeclampsia; 26.7%, severe preeclampsia; 50.3%, and the incidence of eclampsia in toxemia is 23.0%, antepartum eclampsia; 12.8 %, intrapartum eclampsia; 6.9%, postpartum eclampsia; 3. 3%.
3. The mean age of mother is 29. 0.
4. The mean duration of pregnancy is 39.6 weeks.
5. The incidence of primipara in toxemia is 47. 8%, para 2; 17.8%, para 3; 11.9%, para 4; 9.7%, para 5; 5.3%, para over 6; 7.5%.
6. Thee incidence of toxemia in twin pregnancy is 82.6%.
7. The edema (??) in toxemia is 2.8%, edema (trace); 10.1%, edema (??); 42.2%, edema (??); 27.9%, edema (??); 12.3%, edema (??); 21.3%.
8. The proteinuria (??) in toxemia is 12.9%, proteinuria (trace); 7. 9%, proteinuria (??); 26.1%, Proteinuria (??); 19. 5%, proteinuria (??);- 12. 3%, proteinuria (??); 21.3%.
9. The mean systolic blood pressure in antepartum or intrapartum is 173 mmHg.
10. The mean systolic blood pressure on the second day is 140.5 mmHg.
11. The case that showed edema only is 3.1%, proteinuria only; 5.4%, hypertension only; 3.8%, edema plus hypertension; 11. 3%, edema plus proteinuria; 5. 4%, edema plus hypertension plus proteinuria; 63. 5%.
12. The mean hemoglobin concentration in antepartum or intrapartum is 10.5g%.
13. The number of convulsions in eclampsia are as follows;
1 time-36.9%, 2 times-12.9%,
3 times-15. 1%, 4 times-12.7%,
5 times-7.0%, over 6 times-16.4%.
14. The fundoscopic findings are as follows (K-W);
Normal-57. 9%, Grade Ⅰ-11.5%, Grade Ⅱ-18.9%,
Grade Ⅲ-6.8%, Grade Ⅳ-5.8%.
15. The method of delivery is as follows; spontaneous vaginal delivery-79.9%, induced vaginal delivery-9. 0%, Cesarean section-] 0. 5%.
16. The average weight of the newborn babies is 2960 g.
17. The sum of stillbirth and newborn death rates during admission is 8.5%. 18. The maternal death rate is 0.9%.
